Say the word
“Begin dictation.” Otto hears it through AirPods or other microphone devices. Nothing to touch, nothing to distract from the case.

Narrate the way you already do for the room. Implants, findings, blood loss, closure. Say “stop dictation” when you are done.

Review the draft
By the time you have scrubbed out and washed your hands, a structured draft is on your phone. You edit, you sign.
The reason to trust the draft is that it shows its work
An operative note is a legal record with your name on it. Otto is built so you can check it in the time it takes to read it.
It does not fill in the blanks
If you did not say the estimated blood loss, the field stays empty. Otto never infers a typical value, and never writes a sentence you did not support.
Every line shows its source
Each populated field carries the verbatim words from your dictation that produced it. You are checking a transcript against a draft, not trusting a black box.
Nothing is filed without you
The output is a draft. You review it, change what you want, and sign it. Otto has no connection to your EHR and cannot submit anything on your behalf.
The audio does not linger
Recordings are encrypted, used to produce your draft, and deleted on a fixed schedule. They are never used to train a model.
